CLASSE New User Request Form

CLASSE computer accounts must be requested by a member of CLASSE, and they are typically created within two business days after this form is submitted. If a new computer is also required, please fill out the NewComputerForm.

The form gets sent as an email message to members of the CLASSE computer group and is logged in our ticketing system. The information you provide is intended strictly for the Lab's internal use. It will not be supplied to anyone else on a routine basis, although it may be necessary to disclose it in case of problems.

After submitting the form, you should receive an automated confirmation email. If you do not receive any confirmation, then your request was not submitted successfully. Please try again or email service-classe@cornell.edu directly.

When the account has been created, the new user will receive a notice similar to the Welcome to CLASSE message.

If the new user needs a computer, please fill out the Computer Request form too.

Explanations:

  • Basic Information
  • The names and email addresses of the supervisor and new user are needed so that computer account creation notifications can be sent, Replicon can be configured properly and clarification of needs can be requested.

    Warning: For Cornell employees and students, it is not be possible to create a CLASSE computer account until a Cornell NetID has been assigned. Among other things, this will be so email will work correctly. Non-Cornellians, however, do not need a NetID.


  • Research Group
  • The group is needed for computer usage accounting and access to computing resources. In contrast, the WBS number is for charging the time we spend on account and computer setup.

  • Type of Appointment
  • E.g. permanent staff, postdoc, grad student, visitor. We need to know the type of appointment so that the duration of the computer account can be determined.


  • Start Date
  • In order to appropriately schedule the account, we must know the date when the new user will begin working at CLASSE. "ASAP" is meaningless: it means different things to different people.


  • User E-Mail address
  • This should be an e-mail address that new users can read right now in order to receive the notices about computer account creation. New Cornell employees must also activate their NetID e-mail and its forwarding. While CLASSE is happy to e-mail general notices wherever you like, Cornell users with a NetID should use their Cornell E-mail. Cornell Policy requires that work related e-mail be stored and accessed on Cornell's mail service. Many CLASSE notices go to netid@cornell.edu even if we have another address on record. "Non-Cornellians" will be contacted at the E-mail address entered here.
