Instructions:

This form is intended for projects that:

• Consume greater than 8 hours of effort
• Consume less than 8 hours of effort, but involve several stakeholders
• Requires coordinated commissioning
• Total coast (labor plus materials) is under $10k

1. Populate the form with the details of the assignment
2. Review the completed form with the requestor and all stakeholders
3. Schedule at least one check-in with the requestor and stakeholders midway through the project
4. Schedule additional check-ins as needed (e.g. to discuss scope creep, delays, etc.)
5. Update the form as needed for changes in scope, deadlines, delays, or commissioning contraints.
